Turmeric Can Destroy Your Liver — But Only If You Have This One Gene

Turmeric, Curcumin, and Liver Injury: The One Gene That Predicts Who Is at Risk

Turmeric and curcumin supplements feel like the safest thing on the shelf: ancient, natural, anti-inflammatory. For most people that holds up. But for a small group, including many cancer patients who add supplements to their routine, curcumin can trigger serious drug induced liver injury, and which group you fall into is written in your DNA. Here is what the research shows, and why this is one of the few supplement interactions you can actually see coming.

A real case, and a pattern

A healthy 35-year-old woman took an over-the-counter turmeric supplement to recover from workouts. Two months later she was jaundiced, with liver enzymes above 2,000 and bilirubin ten times normal. Every test for viral hepatitis was negative. She stopped the turmeric and nothing else, and her liver recovered over three months. She was not a one-off. The US Drug-Induced Liver Injury Network reported ten turmeric cases, half hospitalized and one fatal. Seven of the ten, including every hospitalized patient, carried the same immune gene variant, HLA-B*35:01.

Why one gene changes the story

HLA genes work like display cases, showing your immune system fragments of what is inside your cells. In carriers of this variant, curcumin and its breakdown products can sit in that display case in a shape the immune system reads as foreign, and T cells then attack liver tissue. This is why the injury is called idiosyncratic. It is driven by genetics, not dose, and it is not classic poisoning.

Why it gets missed in cancer care

Early symptoms are fatigue, nausea, and poor appetite, which is exactly what many people feel during treatment. Lab results can mimic autoimmune hepatitis, so if the supplement is never mentioned, the wrong cause gets treated. For someone on immunotherapy such as Keytruda or Opdivo, that can mean stopping a drug that is working. The good news is that stopping the supplement early usually reverses the damage. Pushing through jaundice is where the real danger begins.

Not just turmeric

Green tea extract in concentrated pill form and He Shou Wu have been linked to the same allele. Resveratrol and berberine have not been convincingly tied to this kind of injury, though berberine has its own drug interaction problem. Ashwagandha has liver injury reports too, through a different and unknown mechanism.

What to do with this

Because HLA type is fixed for life, it can be read from many genomics reports that cancer patients already have. Ask your oncologist whether your HLA type is on file and whether you carry B*35:01 before taking turmeric, green tea extract, or He Shou Wu. If you take any of them and notice dark urine, yellow eyes, or unusual fatigue, stop the supplement and call your care team right away.
